Volunteer Medical Case Management

Case managers are available to assist consumers with a variety of services, including: applying for benefits, gaining access to medical care and medications, referrals to legal professionals, and assistance with transportation. Case Management Program is a unique model that enables consumers to receive services from the same case manager both in the community and at health facilities where they receive their clinical care.
