Case managers are available to assist consumers with a variety of services, including: applying for benefits, gaining access to medical care and medications, referrals to legal professionals, and assistance with transportation.
Case Management Program is a unique model that enables consumers to receive services from the same case manager both in the community and at health facilities where they receive their clinical care.
Case managers address both the medical and social needs of consumers through the following:
- Use of bilingual/bicultural staff, who can serve both as interpreters and as "cultural brokers".
- Integration of CCHER staff into existing hospital clinical teams i.e., Boston Medical Center, Hyde Park Health Associates (HPHA)
- Assignment of CCHER staff to focus on collaboration with private physicians and neighborhood health centers.
- Integration with other CCHER services like mental health and substance abuse
- Accessible and Flexible to consumers at different point of entry
