In response to CCHER focus group report conducted in the town of Delmas from 2007 to 2008, the integration of social services in our health maintenance program is quite necessary for the population we are serving.

MISSION
To integrate of our social services component into our community-based primary care model and to provide the medical and behavioral team the most current living situation about their clients and families who are living with in the greater Delmas area and surrounding communities.

We know that many clients and their families are facing financial hardship. Our social services offer an opportunity to discuss their current problems and when we can, together with client s offer a plan to address their needs.

We offer social services:

• Case management for CCHER clients and their families
  
• Serve as link between clients and medical/behavioral team
  
• Provide short term education and counseling for medical and behavioral clients
  
• Refer clients to Prevention & Education Team for long term and continuing health education & prevention
  
• Referral to other agencies
  
• Family support
  
• Advocacy
  

Our focus is to serve underserved and no income clients and their families.
