PRIMARY MEDICAL CARE SERVICES

A series of focus group was conducted by CCHER in the town of Delmas from 2007 to 2008. The report from these focus groups provided us a conceptual framework to the types and kinds of services to offer to the clients from the Delmas community.

Our community-based primary care service is based on health maintenance model with a strong counseling and education. Our focus is to serve underserved and no income clients and their families. Our disease prevention and health screening have four components

1. Health Maintenance for children under 13

2. Health Maintenance during Adolescence

3. Health maintenance for the male and female Adults

4. Health Maintenance for the Elderly

Results from the prevention and medical interventions from the above components will set the future direction for the expansion of our clinic.

Each client at CCHER clinic works with a team of provider which includes a medical doctor, a nurse, prevention case manager, who are responsible for your care. Your medical doctor serves as a team leader and responsible for all routine medical care you need including exam, laboratory test, diagnosis and treatment. Working together, your nurse and your prevention case manager can address your emotional and other stress factors that affect your life.

Specialty Care and Referrals
CCHER medical provider may refer you to outside medical specialists. If a required specialty is not available at CCHER clinic, your medical provider will refer you to another specialist. Each client is responsible to pay out of pocket for his or her own visit.
